• Mother’s Discharge Certificate: Issued by the hospital where the child was born.
• Child’s Medical Fitness Certificate: Must include the license or registration number of the certifying doctor.
• Legal Dispute Documentation (if applicable): Relevant court documents or orders if there is an ongoing legal matter.
• Single Parent Documentation (if applicable): Proof that: One parent is no longer alive or there has been an irrevocable separation with no second marriage. An affidavit is required for verification.
